in the heart of San Francisco. Our pharmacy is independently owned and
values customer service. We pledge to assist your patients and communicate
with you regarding all of their prescription requirements. Our goal is
to establish a traditional patient care practice.
New prescriptions:
•
Make several copies of the Prescription
Fax Form.
• Complete the form and fax it and the insurance card to us whenever
you write a prescription.
• Have the patient call us to arrange for delivery (San Francisco
only), pickup or mail-away.
Chronic prescriptions: Call us and leave a message for
refill authorization. You may also fax us the refill authorization.
